Early Ischemia and Ionic ImbalanceWithin minutes of spinal cord injury, a secondary cascade begins, progressing over hours to weeks. Summary

Blindness in critically ill patients, often linked to trauma, can result from ischemic optic neuropathy. Prolonged low blood pressure and a sharp decrease in hematocrit are key risk factors.

Background:

  • Ischemic optic neuropathy (ION) is an uncommon cause of blindness in critically ill patients.
  • It is frequently observed in multiple trauma victims without direct ocular injury.

Observation:

  • A case of a young male patient who developed total bilateral blindness is presented.
  • This occurred after severe hemorrhagic shock secondary to thoracoabdominal non-penetrating crush trauma.

Findings:

  • Literature review identified prolonged arterial hypotension as a significant risk factor for ION.
  • A sudden drop in hematocrit value was also identified as a critical risk factor.

Implications:

  • Understanding these risk factors can aid in the early detection and prevention of ION in trauma patients.
  • This case highlights the importance of managing hemodynamic stability and hematological parameters in critically ill trauma survivors.
