Summary

Myringoplasty significantly improves hearing in patients with chronic suppurative otitis media (CSOM-TT). Most patients experienced substantial hearing gain and a reduced air-bone gap after the procedure, demonstrating its effectiveness.

Area of Science:

  • Otolaryngology
  • Surgical Innovation
  • Audiology

Background:

  • Chronic suppurative otitis media of tubotympanic type (CSOM-TT) often requires surgical intervention.
  • Myringoplasty is a key surgical technique for managing tympanic membrane perforations in CSOM-TT.
  • Indications include intermittent discharge and conductive hearing loss, with the goal of closing perforations and enhancing hearing.

Purpose of the Study:

  • To evaluate the effectiveness of myringoplasty in improving hearing function.
  • To assess hearing outcomes within a ten-week postoperative period.

Main Methods:

  • A cohort of 50 patients with CSOM-TT underwent myringoplasty using the underlay technique.
  • Preoperative and postoperative clinical and audiological assessments were performed.
  • The air-bone (A-B) gap was calculated at 500, 1000, and 2000 Hz using temporal muscle fascia grafts.

Main Results:

  • Preoperatively, 76% of patients had an A-B gap of 30 dB or more; postoperatively, only one patient had such a gap.
  • Hearing gain exceeding 15 dB was achieved by 78% of patients.
  • 84% of patients achieved a postoperative A-B gap within 20 dB.

Conclusions:

  • Myringoplasty is an effective surgical procedure for improving hearing in patients with CSOM-TT.
  • A significant reduction in the air-bone gap was observed post-surgery.
  • The study demonstrates substantial hearing improvement in a high percentage of patients undergoing myringoplasty.
