    Background:

    • The fetal pituitary-adrenal axis is critical for regulating fetal development.
    • Understanding adrenocorticotropic hormone (ACTH) secretion patterns is essential for assessing fetal well-being.

    Purpose of the Study:

    • To investigate fetal ACTH levels in amniotic fluid across different gestational ages.
    • To determine the influence of fetal sex, uterine contractions, and maternal complications on these levels.

    Main Methods:

    • Radioimmunoassay was used to measure ACTH in 109 amniotic fluid samples.
    • ACTH levels were analyzed in relation to gestational age, fetal sex, uterine contractions, and maternal complications.
    • ACTH levels in newborn infant urine were compared to amniotic fluid levels.

    Main Results:

    • Significantly higher amniotic fluid ACTH levels were observed between 26 to 30 weeks of gestation compared to early and late pregnancy.
    • Fetal sex, uterine contractions, and maternal complications did not significantly impact amniotic fluid ACTH levels.
    • ACTH levels in the first urine of newborns approximated those in late-pregnancy amniotic fluid.

    Conclusions:

    • Results support a fetal origin for ACTH found in amniotic fluid.
    • Elevated ACTH secretion in the early third trimester may stimulate adrenal cortex development.
    • This stimulation could lead to increased cortisol secretion, vital for fetal lung maturation.
