Summary
Achieving urinary continence in epispadias-exstrophy complex patients is difficult. Vesical neck reconstruction, with or without additional procedures, offers an acceptable continence rate for these patients.
Area of Science:
- Pediatric Urology
- Reconstructive Surgery
- Genitourinary Development
Background:
- The epispadias-exstrophy complex presents significant challenges in achieving urinary continence.
- Management often requires complex surgical interventions to restore bladder and urethral function.
Purpose of the Study:
- To evaluate the effectiveness of vesical neck reconstruction in managing urinary continence for patients with the epispadias-exstrophy complex.
- To determine the success rates of primary and secondary surgical approaches.
Main Methods:
- Retrospective review of 22 pediatric patients who underwent vesical neck reconstruction between 1971 and 1989.
- Analysis of continence outcomes following primary vesical neck reconstruction and subsequent additional/adjunctive procedures.
Main Results:
- Satisfactory urinary continence was achieved in 77% (17 of 22) of patients.
- 8 patients (36%) achieved continence after a single procedure, while 9 of 13 required additional or adjunctive interventions.
- Additional procedures included revision of vesical neck reconstruction, bladder augmentation, and artificial urinary sphincter placement.
Conclusions:
- Vesical neck reconstruction is a viable option for improving urinary continence in the epispadias-exstrophy complex.
- A significant proportion of patients may require staged or combined surgical approaches to achieve optimal continence outcomes.
