Background:

  • Open-angle glaucoma (OAG) is a leading cause of irreversible blindness worldwide.
  • Early detection through effective screening is crucial for timely intervention and vision preservation.
  • Numerous diagnostic tests exist, but their comparative accuracy for OAG screening remains unclear.

Purpose of the Study:

  • To systematically assess and compare the diagnostic accuracy of various screening tests for open-angle glaucoma (OAG).
  • To identify which tests, if any, are most effective for identifying OAG in individuals over 40 years of age.

Main Methods:

  • A comprehensive literature search was conducted across major databases (Medline, Embase, Biosis, Science Citation Index, Cochrane Library) up to late 2005.
  • Studies reporting true and false positives/negatives for OAG screening tests in adults >40 years were included.
  • Meta-analysis was performed using a hierarchical summary receiver operating characteristic (HSROC) model to synthesize data.

Main Results:

  • Forty studies involving over 48,000 participants evaluated nine different tests.
  • Frequency-doubling technology (FDT) showed higher sensitivity than ophthalmoscopy and Goldmann applanation tonometry (GAT).
  • Threshold standard automated perimetry (SAP) and Heidelberg Retinal Tomograph (HRT II) were also more sensitive than GAT, but GAT was more specific. Ophthalmoscopy, SAP, retinal photography, and GAT performed poorly as standalone tests.

Conclusions:

  • No single screening test or combination of tests emerged as clearly superior for detecting open-angle glaucoma.
  • The data quality was heterogeneous, leading to considerable uncertainty in the findings.
  • Further research is essential to rigorously evaluate the comparative accuracy of promising tests like FDT and HRT II for OAG screening.
