Nonaneurysmal nonperimesencephalic subarachnoid hemorrhage: is it a benign entity?

Patients with nonaneurysmal perimesencephalic non-aneurysmal subarachnoid hemorrhage (n-PMN SAH) have a good prognosis and benign clinical course. A second DSA is crucial to rule out aneurysms and vascular lesions.

Background:
- Limited data exists on nonaneurysmal perimesencephalic non-aneurysmal subarachnoid hemorrhage (n-PMN SAH).
- This study analyzes and compares clinical characteristics of n-PMN SAH with PMN SAH and aneurysmal SAH.
Purpose of the Study:
- To analyze the clinical characteristics of n-PMN SAH.
- To compare n-PMN SAH with PMN SAH and aneurysmal SAH.
- To evaluate the outcomes of patients with n-PMN SAH.
Main Methods:
- Patients with spontaneous SAH and negative initial DSA/CTA underwent repeat imaging.
- A second DSA was performed 4-6 weeks later if initial results were negative.
- Patients were classified into PMN SAH and n-PMN SAH groups if no aneurysm was found after two investigations.
Main Results:
- Out of 61 patients with negative initial imaging, 18 had PMN SAH and 34 had n-PMN SAH after exclusions.
- No mortality was observed in the PMN SAH and n-PMN SAH groups.
- At 1.8 years follow-up, 100% of PMN SAH and 94.1% of n-PMN SAH patients had a good outcome.
Conclusions:
- n-PMN SAH patients have a good prognosis and benign clinical course, similar to PMN SAH.
- A second DSA is essential to exclude aneurysms or other vascular lesions.
- Associated comorbid illnesses were more frequent in PMN SAH and n-PMN SAH compared to aneurysmal SAH.
