Background:

Apocrine hidrocystomas are benign cystic tumors arising from apocrine sweat glands.
These lesions commonly present as translucent papules, often on the face.

Observation:

An 83-year-old woman presented with a 7-year history of translucent papules on the nose, peri-orbital region, and cheeks.
Lesions were noted to worsen with heat and exercise.

Findings:

Histopathologic examination revealed thin-walled cysts lined by flattened, bi-layered epithelium.
Decapitation secretion was observed in the upper epidermis, consistent with apocrine origin.

Implications:

The diagnosis of multiple apocrine hidrocystomas was established based on clinical and histopathological evidence.
