Background:

  • Malignant histiocytosis is a rare hematologic disorder characterized by systemic proliferation of histiocytes.
  • Hepatic involvement can occur in various hematologic malignancies, presenting diagnostic challenges.

Observation:

  • A patient presented with fever, anemia, thrombocytopenia, coagulopathy, and elevated liver enzymes, suggestive of malignant histiocytosis.
  • Liver biopsy revealed a sinusoidal infiltrate of neoplastic cells, initially raising suspicion for malignant histiocytosis.
  • Subsequent development of supraclavicular lymphadenopathy led to a diagnosis of diffuse large-cell lymphoma.

Findings:

  • Immunophenotypic studies confirmed a B-cell phenotype for the neoplastic cells in both the liver biopsy and lymph node.
