Background:

Uterine leiomyomas (fibroids) are common benign tumors affecting women of reproductive age.
Diffuse uterine leiomyomatosis is a rare variant with potential for rapid growth.
Hormonal therapies are often used to manage symptomatic fibroids.

Observation:

A 28-year-old woman presented with primary infertility and severe menorrhagia due to multiple uterine leiomyomas.
Hysterectomy was performed after hormonal therapy failure.
Pathologic findings confirmed diffuse uterine leiomyomatosis with features similar to typical leiomyomas.

Findings:

The diffuse leiomyomatosis demonstrated histologic, immunohistochemical, and ultrastructural similarities to common uterine leiomyomas.
Rapid tumor growth correlated with the administration of clomiphene citrate, an estrogenic agonist.
Intralesional hemorrhage occurred during norethindrone acetate treatment following clomiphene cessation.

Implications:

This case suggests a potential role for estrogenic agents like clomiphene citrate in the rapid proliferation of diffuse uterine leiomyomatosis.
Understanding hormonal influences is crucial for managing rare uterine leiomyoma variants.
Further research is needed to elucidate the specific mechanisms driving diffuse leiomyomatosis growth and response to hormonal agents.
