A rare cardiac myxoma presenting as a large, thin-walled cystic mass filled with arterial blood was found in a 38-year-old woman's right atrium. This unique presentation required advanced imaging and surgical intervention.

Area of Science:

  • Cardiovascular Pathology
  • Cardiac Oncology

Background:

  • Cardiac myxomas are the most common primary heart tumors, typically originating in the left atrium.
  • Cystic variants and right atrial location are exceptionally rare, posing diagnostic challenges.

Observation:

  • A 38-year-old woman presented with a large (6 x 5 x 4.5 cm), thin-walled, cystic mass in the right atrium.
  • The mass was filled with arterial blood and clearly visualized using transesophageal echocardiography and cardiac MRI.
  • Intraoperatively, a smooth, silver-whitish cystic mass was identified on the right atrial free wall.

Findings:

  • Microscopic examination revealed a cystic wall composed of stellate mesenchymal cells within a myxoid matrix, confirmed by alcian blue staining.
  • Histological analysis supports a diagnosis of cardiac myxoma with unusual cystic and vascular characteristics.

Related Experiment Videos

Implications:

  • This case highlights a novel cystic presentation of cardiac myxoma in a rare location.
  • It underscores the importance of comprehensive imaging and histological evaluation for accurate diagnosis of atypical cardiac tumors.
