Sleep Apnea01:21

Sleep Apnea

Sleep apnea is a condition where breathing stops intermittently during sleep, often leading to significant health issues. Each episode can last from 10 to 20 seconds or more and is frequently accompanied by a brief arousal from sleep. This disturbance, largely unnoticed by the individual, can lead to severe daytime fatigue. Commonly, individuals seek help after being informed by their partners about loud snoring and noticeable breathing pauses during sleep.
The condition is more prevalent among...

Pharmacokinetics in Geriatric Patients: Effect of Age on Drug Excretion

In geriatric patients, renal physiology undergoes significant changes, including diminished renal blood flow and a lower glomerular filtration rate (GFR), leading to alterations in medication clearance. Drugs such as aminoglycoside antibiotics, lithium, and digoxin, which rely on glomerular filtration for removal from the body, particularly impact pharmacokinetics. These drugs tend to have slower clearance rates in older adults, necessitating careful dosage considerations.Evaluation of renal...
REM Sleep Behavior Disorder01:15

REM Sleep Behavior Disorder

REM Sleep Behavior Disorder (RBD) is a sleep disorder characterized by the absence of muscle paralysis that normally occurs during the REM phase of sleep. This absence allows individuals to physically act out their dreams, which are often vivid and disturbing. Common behaviors exhibited during episodes include kicking, punching, and yelling. These actions can be dangerous, potentially leading to injuries for the person with RBD or their bed partner.
RBD is significantly associated with...
Insufficient Sleep and Sleep Deprivation01:13

Insufficient Sleep and Sleep Deprivation

Insufficient sleep refers to not getting the recommended amount of sleep for optimal functioning, even if it's just slightly less than needed. Sleep insufficiency may occur due to lifestyle choices, such as staying up late for social events or work, resulting in routinely getting less sleep than required. For example, consistently sleeping 6 hours when the body needs 7-9 hours can lead to cumulative effects on health and well-being.
Sleep deprivation is a more severe form of sleep loss...
Pharmacodynamics in Geriatric Patients: Effects of Age01:27

Pharmacodynamics in Geriatric Patients: Effects of Age

Age-related pharmacokinetic changes are extensively documented, but understanding age-related pharmacodynamic alterations is relatively limited. This knowledge gap can be partly attributed to the complexity of developing appropriate measures of drug responses compared to bioanalytical methods for determining drug concentrations. Most information regarding age-related differences in human pharmacodynamics originates from cross-sectional studies. However, these studies assume that observed mean...

Heart Failure VI: Adjunct Therapies

Additional therapies for treating patients with heart failure (HF) may include procedural interventions, supplemental oxygen, the management of sleep disorders, and nutritional therapy.Procedural InterventionsImplantable Cardioverter-Defibrillator: For patients at risk of life-threatening arrhythmias due to severe left ventricular dysfunction, an Implantable Cardioverter-Defibrillator (ICD) can detect and terminate these arrhythmias, preventing sudden cardiac death and improving survival rates.

Summary

Reduced kidney function in older men was not linked to sleep-disordered breathing (SDB). The association observed in younger old men was primarily due to higher body mass index, not kidney disease itself.

Area of Science:

  • Nephrology
  • Sleep Medicine
  • Geriatrics

Background:

  • Sleep-disordered breathing (SDB) is prevalent in severe chronic kidney disease (CKD), potentially increasing morbidity and mortality.
  • The relationship between mild to moderate CKD and SDB risk remains unclear.
  • Investigating SDB in CKD is crucial for managing patient outcomes.

Purpose of the Study:

  • To examine the association between reduced renal function and SDB in older men.
  • To determine if mild to moderate CKD influences the likelihood of SDB.
  • To explore potential confounding factors in this relationship.

Main Methods:

  • Utilized data from 2696 men aged 65+ from the MrOS Sleep study.
  • Assessed chronic kidney disease (CKD) using estimated glomerular filtration rate (eGFR) calculated from serum creatinine.
  • Diagnosed sleep-disordered breathing (SDB) via respiratory disturbance index (RDI) and oxygen desaturation during polysomnography (PSG).

Main Results:

  • No significant association was found between lower eGFR and higher RDI in the overall cohort.
  • An interaction between eGFR and age suggested a potential link in younger old men (
  • This association in younger old men was not statistically significant after adjusting for body mass index (BMI).

Conclusions:

  • Reduced renal function in older men is not independently associated with increased SDB.
  • Higher BMI appears to be a significant factor mediating the link between lower eGFR and SDB in younger old men.
  • Further research in younger populations is warranted to fully elucidate these findings.
