Purpose of the Study:

  • To evaluate if competitions and incentives increase long-term smoking cessation rates.
  • To examine the correlation between incentive programs and participation rates.

Main Methods:

  • Systematic review of randomized controlled trials and controlled studies.
  • Searched multiple databases (Cochrane Tobacco Addiction Group, MEDLINE, EMBASE, CINAHL, PsycINFO) up to December 2007.
  • Primary outcome was smoking abstinence at six months, using biochemically validated rates where available.

Main Results:

  • Seventeen studies met inclusion criteria; none showed significantly higher long-term quit rates for incentive groups.
  • No clear evidence indicated that self-funded participation or specific incentive types were more effective.
  • Incentives may improve recruitment rates, potentially increasing the overall number of quitters.

Conclusions:

  • Incentives and competitions have not demonstrated efficacy in enhancing long-term smoking cessation.
  • Observed early success often diminishes once rewards are removed.
  • Rewarding participation and compliance shows potential for increasing the absolute number of individuals who quit smoking.
