New hemostatic technologies (NT) in thyroid surgery showed a shorter operation time with the Harmonic Scalpel (HS). However, both HS and Ligasure (LI) were associated with higher morbidity compared to conventional techniques (CT).

Area of Science:

  • Surgical Innovation
  • Thyroid Surgery
  • Hemostasis

Background:

  • New hemostatic technologies (NT) are increasingly used in thyroid surgery.
  • Their effectiveness in reducing operating time and complications is under investigation.

Purpose of the Study:

  • To compare the efficacy of three hemostatic techniques in thyroidectomy.
  • Specifically, conventional technique (CT), Ligasure vessel sealing system (LI), and Harmonic Scalpel (HS).

Main Methods:

  • A prospective randomized study involving 150 patients undergoing total thyroidectomy.
  • Patients were assigned to CT, LI, or HS groups.
  • Demographics and outcomes were recorded.

Main Results:

  • Harmonic Scalpel (HS) significantly reduced operation time (p < 0.001).
  • Morbidity was higher in new hemostatic technology (NT) groups (HS and LI) compared to conventional technique (CT) (p = 0.0002).
  • NT patients exhibited significantly lower postoperative calcemia (p < 0.05).
  • Conclusions:

    • The primary advantage of new hemostatic technologies (NT) in thyroid surgery is a reduced operation time, particularly with the Harmonic Scalpel (HS).
    • Higher morbidity rates were observed with both HS and LI compared to conventional techniques.
