Summary

Patients with chronic kidney disease (CKD) have significantly reduced physical functioning before starting dialysis. Interventions are needed to improve physical function in CKD patients prior to renal replacement therapy (RRT).

Background:

  • Limited data exists on the physical functioning status of CKD patients before initiating renal replacement therapy (RRT).

Purpose of the Study:

  • To assess the physical functioning of pre-dialysis CKD patients using objective and subjective measures.
  • To investigate the relationship between physical functioning parameters and renal function in this cohort.

Main Methods:

  • Thirty-two pre-dialysis CKD patients underwent a symptom-limited treadmill test for peak oxygen uptake (VO2peak).
  • Physical performance was evaluated through gait speed, sit-to-stand tests, and the 6-minute walk test.
  • Self-reported physical functioning was assessed using the SF-36 questionnaire.

Main Results:

  • Pre-dialysis CKD patients demonstrated reduced VO2peak, physical performance, and self-reported functioning compared to age-predicted norms.
  • VO2peak correlated significantly with all assessed physical functioning measures.
  • Maximal gait speed and the physical composite scale (PCS) showed significant correlations with estimated glomerular filtration rate (eGFR).

Conclusions:

  • Pre-dialysis CKD patients exhibit objectively and subjectively impaired physical functioning.
  • Low physical functioning is a predictor of outcomes in dialysis patients, underscoring the need for interventions.
  • Maintaining or enhancing physical function before RRT initiation is crucial for CKD patient management.
