Bones of the Upper Limb: Humerus01:19

Bones of the Upper Limb: Humerus

The upper limb consists of the arm, forearm, wrist, and hand bones. The humerus is the single bone of the upper arm region. Proximally, it has a large, spherical, smooth head that articulates with the glenoid cavity of the scapula to form the glenohumeral or shoulder joint. The margin of the head is the anatomical neck, a residual epiphyseal plate. Laterally it extends to form bony projections called the greater tubercle and the lesser tubercle. Next to the tubercles is the surgical neck, a...

Summary
This summary is machine-generated.

This study presents a novel surgical technique for patients needing both shoulder and elbow replacements due to arthritis. Custom interlocking prostheses with an internal strut successfully prevented fractures in a rheumatoid arthritis patient.

Area of Science:

  • Orthopedic Surgery
  • Biomedical Engineering

Background:

  • Ipsilateral shoulder and elbow arthritis frequently requires surgical intervention.
  • Simultaneous or sequential arthroplasty in both joints poses a risk of periprosthetic fracture due to the stress riser effect between implants.

Observation:

  • A patient with rheumatoid arthritis presented with severe ipsilateral shoulder and elbow arthritis requiring arthroplasty.
  • The surgical approach involved staged implantation of an uncemented elbow prosthesis followed by a custom-designed shoulder prosthesis.

Findings:

  • A custom internal strut was integrated between the shoulder and elbow prostheses to mitigate stress concentration.
  • At seven years postoperatively, the patient remained asymptomatic with no radiographic evidence of implant failure or fracture.

Implications:

  • This technique offers a potential solution for managing complex cases of ipsilateral shoulder and elbow arthritis.
  • Custom interlocking prostheses with internal strutting may reduce the risk of periprosthetic fractures in combined upper extremity arthroplasty.
