Summary

Endoscopic sphincterotomy (ES) is a highly effective treatment for acute obstructive cholangitis (AOC), rapidly improving symptoms in most patients. This minimally invasive procedure should be the primary choice, with surgery reserved for cases where ES fails.

Digestive diseases and sciences·2021

Area of Science:

  • Gastroenterology
  • Hepatology
  • Surgical Endoscopy

Context:

  • Acute obstructive cholangitis (AOC) is a serious biliary tract infection.
  • Diagnosis relies on Charcot's triad, laboratory findings, and imaging.
  • Common causes include choledocholithiasis and papillary stenosis.

Purpose:

  • To evaluate the efficacy of urgent endoscopic sphincterotomy (ES) in managing AOC.
  • To determine the success rate and complication profile of ES for AOC.
  • To establish ES as the preferred treatment modality for AOC.

Summary:

  • Forty-five patients with AOC underwent ES within 24 hours of admission.
  • Common bile duct stones were the primary etiology (84.5%).
  • ES led to rapid clinical improvement in 40 patients (89%), with significant reduction in pain, fever, WBC, and bilirubin levels.

Impact:

  • Urgent ES is a safe and effective first-line treatment for AOC.
  • Successful endoscopic drainage was achieved in 91% of patients.
  • Surgery is indicated only for ES failures, minimizing invasive procedures.
