Background:

  • Inflammatory pseudotumor is a rare condition involving inflammatory cell infiltration and fibrosis.
  • It is exceptionally uncommon to find inflammatory pseudotumor solely within the middle ear.
  • This case highlights the diagnostic challenge it presents, especially when mimicking other middle ear pathologies.

Observation:

  • A 7-year-old girl experienced hearing impairment in her right ear.
  • Initial suspicion of otitis media with effusion led to a myringotomy.
  • During the procedure, a mass distinct from congenital cholesteatoma was discovered.

Findings:

  • Surgical excision via canal wall-down tympanomastoidectomy successfully removed the middle ear mass.
  • Pathologic examination confirmed the mass as an inflammatory pseudotumor.
  • This represents an extremely rare occurrence of this specific pathology in the middle ear.

Implications:

  • This case underscores the importance of considering rare diagnoses in pediatric ear conditions.
  • Accurate pathological diagnosis is crucial for appropriate management of middle ear masses.
  • Further literature review is needed to understand the characteristics and treatment of middle ear inflammatory pseudotumors.
