Summary

Rotationplasty is a viable option for older sarcoma patients, offering an alternative to above-knee amputation. However, functional outcomes may be poorer compared to younger individuals.

Area of Science:

  • Orthopedic Surgery
  • Oncology
  • Geriatric Medicine

Background:

  • Rotationplasty is an established limb-sparing surgery for lower extremity bone tumors, primarily in pediatric and young adult populations.
  • Its application in patients over 60 years old remains largely undocumented.

Observation:

  • This study evaluated three patients over 60 who underwent rotationplasty for lower extremity sarcoma.
  • Complications and functional outcomes were assessed, including electromyography in one case.

Findings:

  • Despite good muscle adaptation shown by electromyography, functional results were limited in this older cohort.
  • Two out of three patients required a cane for distances exceeding 200 meters.
  • No secondary amputations were necessary.

Implications:

  • Rotationplasty can serve as an alternative to above-knee amputation in elderly patients with lower extremity sarcomas.
  • Functional outcomes in older adults may be inferior to those observed in younger patients.
  • Limb-salvage surgery, where feasible, remains the preferred approach, even in older populations.
