Appendectomy is a common surgical procedure for acute appendicitis.
Complications typically manifest within weeks of surgery.

Observation:

A rare case of an appendiceal abscess developed over 18 years after an initial appendectomy.
The abscess presented as a neoplastic obstructive process.

Findings:

A communication pathway was identified between the cecum and the abscess cavity.
This occurred through the remaining appendiceal stump.

Implications:

This case underscores the possibility of late-onset complications from appendiceal stumps.
Delayed abscess formation can mimic malignancy, necessitating thorough diagnostic evaluation.
