Background:

  • Cat scratch disease, caused by Bartonella henselae, is a common zoonotic infection.
  • Ocular manifestations of Bartonella henselae infection can be diverse and severe.

Observation:

  • A 45-year-old patient presented with bilateral panuveitis and serous retinal detachment.
  • Symptoms developed two weeks after experiencing influenza-like illness with lymphadenopathy.
  • A macular star pattern was observed, more prominent in the left eye.

Findings:

  • Serological tests confirmed high levels of IgG and IgM antibodies against Bartonella henselae.
  • Inflammatory markers were significantly elevated.
  • Treatment with sulfonamides and corticosteroids, followed by doxycycline, led to clinical improvement and enhanced visual acuity.

Implications:

  • Bartonella henselae should be considered in the differential diagnosis of panuveitis with serous retinal detachment.
  • Anterior uveitis may precede the development of posterior segment inflammation.
  • Prompt diagnosis and appropriate antibiotic therapy are essential for favorable visual outcomes in ocular Bartonellosis.
