Summary

A rare case of multiseptate gallbladder was incidentally discovered during gastric cancer surgery. Imaging and pathological findings confirmed this rare gallbladder anomaly, distinct from the patient's primary cancer.

Background:

Gastric cancer diagnosis and treatment often involve advanced imaging techniques.
Incidental findings during surgical procedures can reveal rare anatomical variations.

Observation:

A 53-year-old female with asymptomatic gastric cancer presented with an incidentally detected complex cystic gallbladder.
Imaging studies including ultrasonography, DIC-CT, and MRCP revealed a multiseptate gallbladder with multiple linear hyperechogenic septa.
Endoscopic examination showed a normal papilla of Vater, and no dilation of bile or pancreatic ducts was observed.

Findings:

Histopathological examination of the gallbladder, removed during distal partial gastrectomy and cholecystectomy for gastric cancer, confirmed a multiseptate gallbladder.
The gallbladder displayed a honeycomb appearance due to multiple thin septa, with no evidence of malignancy within the gallbladder itself.
The resected stomach tumor was a poorly differentiated adenocarcinoma.

Implications:

This case highlights the importance of recognizing rare gallbladder anomalies incidentally found during investigations for other conditions.
Multiseptate gallbladder, though rare, should be considered in differential diagnoses for cystic gallbladder lesions.
Accurate preoperative imaging and subsequent pathological analysis are crucial for differentiating gallbladder anomalies from neoplastic processes.
