Anatomy of the Adrenal Glands

The adrenal or supra-renal glands, situated above the kidneys and aligned with the twelfth rib, are paired pyramid-shaped structures crucial for the body's stress response. During stress, these glands secrete hormones vital for adaptive physiological reactions.
These glands possess a distinctive yellow tinge due to the stored cholesterol and fatty acids required for hormone synthesis. They are encased in a fibrous capsule and cushioned by fat.
The adrenal gland comprises two distinct regions...

Area of Science:

  • Oncology
  • Pathology

Background:

  • Primary adrenal angiosarcoma is exceedingly rare.
  • Adrenal gland masses can present insidiously, sometimes mimicking other pathologies.

Observation:

  • A 49-year-old female presented with blunt abdominal trauma.
  • Initial imaging suggested a liver neoplasm rupture, but surgery revealed a right adrenal gland mass.
  • The mass involved the adrenal gland and periadrenal fat.

Findings:

  • Microscopic examination showed diffuse neoangiogenesis with large, hyperchromatic cells and widespread necrosis.
  • Histological features were consistent with angiosarcoma, distorting adrenal gland architecture.
  • The tumor was completely excised with locoregional lymph nodes.

Implications:

  • This case expands the differential diagnosis for adrenal masses.
  • Complete surgical resection appears to be an effective initial treatment strategy.
  • Further research is needed to understand the behavior and optimal management of primary adrenal angiosarcoma.
