Summary

Chromoblastomycosis (CBM) is a fungal skin infection. A new clinical type was identified and successfully treated with itraconazole, showing no recurrence after 2 years.

Background:

  • Chromoblastomycosis (CBM) is a chronic fungal infection of the skin.
  • It is characterized by specific fungal structures and varied lesion morphologies.
  • Difficult-to-treat cases pose a clinical challenge.

Observation:

  • Three patients presented with a distinct, large, annular, slow-growing CBM lesion.
  • Lesions were resistant to topical antifungal treatments.
  • Diagnosis was confirmed as CBM caused by Fonsecaea pedrosoi via skin scrapings and biopsies.

Findings:

  • A novel clinical presentation of CBM is described.
  • Treatment with oral itraconazole (200 mg/day) for 6-9 months led to complete lesion regression.
  • No recurrence was observed during two years of follow-up.

Implications:

  • Itraconazole is an effective and safe therapeutic option for this CBM subtype.
  • This study expands the understanding of CBM clinical presentations.
  • Prompt diagnosis and targeted therapy are crucial for successful CBM management.
