Summary

The Mecron threaded-in acetabular cup showed a high failure rate, especially in complex revision surgeries and cases of femoral head osteonecrosis. Success rates were 87% for primary and 59% for revision total hip replacements.

Area of Science:

  • Orthopedic Surgery
  • Biomaterials Engineering
  • Medical Device Technology

Background:

  • Threaded-in acetabular implants offer an attractive fixation method using torque and compression.
  • Long-term fixation of these implants has become a subject of recent controversy.
  • The Mecron hemispheric self-tapering threaded-in acetabular cup was evaluated for its efficacy.

Purpose of the Study:

  • To assess the clinical and radiographic outcomes of the Mecron threaded-in acetabular cup.
  • To determine the success rates of this implant in primary versus revision total hip replacements (THRs).
  • To identify potential factors contributing to implant failure.

Main Methods:

  • A retrospective review of 37 cases (42 hips) treated with Mecron cups between July 1985 and June 1989.

Related Experiment Videos

  • Clinical and radiographic follow-up for a minimum of 2 years (average 39 months) in 29 cases (32 hips).
  • Analysis of outcomes based on primary versus revision arthroplasty, including cases of osteonecrosis of the femoral head.
  • Main Results:

    • The overall success rate was impacted by a higher failure rate in the revisional group (59%) compared to the primary group (87%).
    • Fifteen primary THRs included 10 cases of osteonecrosis of the femoral head.
    • A significant percentage of surgeries were revisional (53%), potentially influencing the observed failure rates.

    Conclusions:

    • The Mecron threaded-in cup demonstrated a relatively high rate of failure in this patient cohort.
    • The high proportion of complex cases, including revision surgeries and osteonecrosis, likely contributed to the suboptimal outcomes.
    • Further investigation into implant design and patient selection for threaded acetabular components is warranted.
