Asymptomatic type 2 diabetes patients show endothelial dysfunction (ED) detectable by (13)N-ammonia-positron emission tomography (PET). This imaging technique quantifies myocardial blood flow changes, revealing ED even in early disease stages.

- Performed a three-phase (13)N-ammonia-PET scan to measure MBF at rest, during cold pressor test (CPT), and pharmacologic stress.
- Calculated EDVI, MFR, and %DeltaMBF, normalizing values to the rate-pressure product (RPP) for corrected EDVI' and %DeltaMBF'.

- Diabetic patients exhibited significantly lower mean EDVI' (0.864 vs. 1.110, p=0.004) and %DeltaMBF' (-8.2% vs. 4.5%, p=0.005) compared to healthy controls.
- While MFR showed no significant difference (p=NS), the %DeltaMBF was markedly reduced in diabetic individuals (20% vs. 55%, p=0.002).
- These findings indicate impaired endothelial-dependent vasodilation in asymptomatic type 2 diabetes.
Conclusions:
- Asymptomatic, recently diagnosed type 2 diabetes patients demonstrate significant endothelial dysfunction.
- (13)N-ammonia-PET is a valuable tool for quantifying ED in this patient population.
- Early detection of ED using PET can inform risk stratification and management strategies for type 2 diabetes.
