Purpose of the Study:

  • To examine morbidity and mortality in adult asthma patients requiring mechanical ventilation over a 10-year period.
  • To review the clinical features and management of these critically ill asthma patients.

Main Methods:

  • Retrospective review of medical records for adult patients with asthma requiring mechanical ventilation.
  • Study conducted over a 10-year period at a university-affiliated county hospital.

Main Results:

  • 127 patients (162 episodes) required mechanical ventilation for asthma; 64% were women, 65% African-American.
  • Patients had risk factors for mortality: prior hospitalizations, near-fatal asthma, non-compliance, poor follow-up.
  • Inhaled corticosteroid use increased from 18% (1990) to 80% (1998); tidal volumes decreased post-1995.
  • Most common complication: atelectasis (33 cases). Barotrauma in 10 cases. Four deaths occurred, all with pre-hospital cardiopulmonary arrest and anoxic brain injury.

Conclusions:

  • No deaths were attributed to mechanical ventilation complications, despite patient risk factors.
  • Improved mechanical ventilation management and outpatient asthma therapy likely contributed to low mortality.
