Summary

Dimeticone, a silicone oil pediculicide, demonstrated superior efficacy and cosmetic acceptability over permethrin for treating head lice infestations in children. Resistance to dimeticone is unlikely due to its physical mode of action.

Background:

  • Head lice (Pediculus humanus capitis) infestations are a global health concern.
  • Increasing resistance to conventional pediculicides necessitates novel treatment strategies.
  • Dimeticone, a silicone oil, is investigated as an alternative treatment.

Purpose of the Study:

  • To compare the efficacy and safety of a 92% dimeticone product against 1% permethrin lotion for treating head lice.
  • To evaluate cure rates, itching reduction, and cosmetic acceptability of both treatments.

Main Methods:

  • A randomized, controlled, observer-blinded clinical trial involving 145 children aged 5-15 years in Brazil.
  • Participants were temporarily relocated to minimize reinfestation.
  • Two applications of dimeticone or permethrin were administered seven days apart.

Main Results:

  • Dimeticone achieved significantly higher cure rates on day 2 (94.5%) and day 9 (97.2%) compared to permethrin (66.7% and 67.6%, respectively).
  • Itching reduction was similar between groups; however, cosmetic acceptability was significantly better with dimeticone.
  • Two mild product-related incidents were reported in the dimeticone group.

Conclusions:

  • The dimeticone product is a safe and highly effective pediculicide for head lice.
  • Its physical mode of action, disrupting lice oxygen supply, makes resistance development unlikely.
  • Dimeticone offers a promising alternative to conventional treatments facing resistance issues.
