Summary

Three cases of epidermal inclusion cysts originating in the tarsus, an unusual location, were identified. These intratarsal cysts, initially mistaken for chalazia, required complete excision for effective treatment.

Background:

  • Epidermal inclusion cysts are typically found in the dermis.
  • The tarsus is an unusual anatomical site for epidermal inclusion cyst development.

Observation:

  • Three cases of intratarsal epidermal inclusion cysts were identified over a decade.
  • These cysts initially presented with symptoms similar to chalazia.
  • Incision and curettage resulted in recurrence in all three cases.

Findings:

  • Histopathology confirmed keratin-filled cysts lined by stratified keratinized epithelium arising from the tarsus.
  • Cysts measured 8-10 mm and contained gelatinous material.
  • Complete excision of the cyst with surrounding tarsus prevented recurrence.

Implications:

  • Intratarsal epidermal inclusion cysts can mimic chalazia, necessitating careful differentiation.
  • Incision and curettage is an ineffective treatment for these tarsal cysts.
  • Surgical excision of the cyst and involved tarsus is recommended for definitive management.
