Drug-Induced Senescence in Liver Cells Promotes M2 Macrophage Polarization: Implications for Tyrosine Kinase Inhibitor-Associated Hepatotoxicity

Background:

  • The non-steroidal anti-inflammatory drug (NSAID) nimesulide has been associated with liver injury.
  • The link between nimesulide and severe hepatotoxicity, specifically fulminant hepatic failure (FHF), remains controversial.

Purpose of the Study:

  • To investigate the association between nimesulide use and the occurrence of FHF.
  • To determine if nimesulide is a potential cause of drug-induced liver injury leading to liver transplantation.

Main Methods:

  • A retrospective cohort study was conducted at the Irish national liver transplant unit.
  • Patients who received liver transplants for FHF of unknown etiology between 1994 and 2007 were evaluated.
  • Medication history, focusing on drugs initiated within six months prior to transplant, was reviewed. Causality was assessed using Naranjo and Roussel UCLAF Causality Assessment Method (RUCAM) scoring systems.

Main Results:

  • Thirty-two patients with sero-negative, non-paracetamol-induced FHF requiring emergency liver transplantation were identified.
  • Nimesulide was identified as a new medication started within six months prior to transplant in six of these patients.
