Summary

Tuberculosis of the sternum is rare. This case highlights successful treatment of sternal osteomyelitis caused by Mycobacterium tuberculosis with anti-tuberculosis drugs and surgical debridement.

Background:

  • Tuberculous osteomyelitis of the sternum is an uncommon presentation of extrapulmonary tuberculosis.
  • Isolated sternum involvement is rare, accounting for less than 1% of all tuberculous osteomyelitis cases.

Observation:

  • A 51-year-old woman presented with a painful, solid mass in the lower sternum.
  • Physical examination revealed a 3 cm soft, painful mass. Radiological imaging showed soft tissue and bone expansion.

Findings:

  • Initial antibiotic treatment for suspected osteomyelitis was ineffective.
  • Microbiological analysis confirmed Mycobacterium tuberculosis in fine needle aspiration fluid.
  • The patient received a four-drug anti-tuberculosis regimen (isoniazid, rifampicin, pyrazinamide, ethambutol).
