Summary

A large retroperitoneal liposarcoma caused breathing difficulties and was successfully removed. Surgical resection and radiation therapy are key treatments for this rare soft tissue sarcoma.

Background:

  • Retroperitoneal soft tissue sarcomas are rare and can grow to significant sizes before diagnosis.
  • Giant abdominal tumors can cause significant complications, including respiratory compromise due to mass effect.

Observation:

  • A 36-year-old male presented with dyspnea and a rapidly growing abdominal mass over 10 weeks.
  • Initial ultrasound was inconclusive, but Magnetic Resonance (MR) imaging successfully characterized the large, inhomogeneous retroperitoneal tumor.
  • The 24.2 kg tumor was found to be adhered to the right kidney capsule.

Findings:

  • The tumor was completely removed (en bloc resection).
  • Histopathological examination confirmed the diagnosis of a sclerosing, poorly differentiated liposarcoma.
  • This histological subtype indicates an aggressive tumor with a potential for recurrence.

Implications:

  • Radical surgical excision is crucial for managing large retroperitoneal liposarcomas.
  • Adjuvant radiation therapy is recommended for retroperitoneal soft tissue sarcomas to improve local control and reduce recurrence rates.
  • Accurate radiological assessment, particularly with MR imaging, is vital for surgical planning and differentiating tumors from adjacent organs.
