Early laparoscopic cholecystectomy for biliary colic significantly reduces complications and hospital stays compared to delayed surgery. This approach also lowers conversion rates to open procedures, offering better patient outcomes.

Background:

Biliary colic is a common reason for laparoscopic cholecystectomy.
Elective surgery involves lengthy waiting periods, increasing risks of severe complications.

Purpose of the Study:

To evaluate the benefits and harms of early versus delayed laparoscopic cholecystectomy for patients experiencing biliary colic due to gallstones.

Main Methods:

A systematic review of randomized clinical trials was conducted, searching databases like MEDLINE and EMBASE until March 2008.
Data extraction and analysis were performed by two independent reviewers, calculating risk ratios and weighted mean differences.

Main Results:

One high-risk bias trial with 75 patients was included. Early surgery (within 24 hours) showed fewer complications during the waiting period, including reduced acute pancreatitis, gallbladder issues, and jaundice.
The early group had a significantly lower conversion rate to open surgery (0% vs. 20%), shorter operating times, and reduced hospital stays.

Conclusions:

Early laparoscopic cholecystectomy appears to reduce morbidity, conversion rates, operating time, and hospital stay for biliary colic patients.
Further high-quality randomized clinical trials are needed to confirm these findings.
