Background:

  • Nissen fundoplication is a common surgical treatment for gastroesophageal reflux disease (GERD).
  • Postoperative symptoms like dysphagia, inability to belch/vomit, and gas bloating are frequently reported after this procedure.

Purpose of the Study:

  • To investigate the long-term incidence and characteristics of dysphagia following Nissen fundoplication.
  • To determine if esophageal motor abnormalities correlate with persistent dysphagia after surgery.

Main Methods:

  • A cohort of 32 patients who underwent Nissen fundoplication 3.5 to 18 years prior were studied.
  • Conventional manometry and 24-hour ambulatory pressure and pH monitoring were utilized to assess esophageal function.
  • Patient-reported dysphagia symptoms were evaluated.

Main Results:

  • Dysphagia symptoms showed a tendency to decrease over time after Nissen fundoplication.
  • Neither conventional nor ambulatory manometry revealed significant motor abnormalities that could explain the dysphagia.
  • No clear correlation was found between esophageal pressure recordings and the presence of dysphagia.

Conclusions:

  • Dysphagia does not appear to be a major long-term complication of Nissen fundoplication.
  • The study data do not support the hypothesis that motor abnormalities are the primary cause of dysphagia after this antireflux surgery.
  • The findings suggest that dysphagia may resolve or diminish in many patients following Nissen fundoplication.
