Elderly patients with primary biliary cirrhosis (PBC) often present asymptomatically. This asymptomatic presentation in older individuals with PBC contributes to similar survival rates compared to younger patients.

Area of Science:

  • Hepatology
  • Gastroenterology
  • Internal Medicine

Background:

  • Primary biliary cirrhosis (PBC) is a chronic cholestatic liver disease.
  • PBC onset typically occurs around menopause.

Purpose of the Study:

  • To investigate the clinical features and natural history of PBC in relation to patient age.
  • To compare outcomes between younger and older PBC patient cohorts.

Main Methods:

  • Retrospective analysis of 86 consecutive PBC patients (81 female, 5 male).
  • Patients were divided into two groups: <65 years (n=70) and >=65 years (n=16).
  • Follow-up duration ranged from 6 months to 16 years (mean 4 years).

Main Results:

  • Histological stage at presentation was similar between age groups.

Related Experiment Videos

  • A significantly higher prevalence of asymptomatic patients was observed in the elderly PBC group (56% vs 24%, p<0.001).
  • No significant differences in biochemical or immunological markers were found. Survival rates were comparable, with mortality only in the <65 years group (21.4%).
  • Conclusions:

    • The high proportion of asymptomatic elderly PBC patients contributes to similar survival rates across age groups.
    • Age does not appear to be a significant factor in PBC prognosis, likely due to earlier detection in asymptomatic older individuals.
