Summary

Choledochal cysts are often found during gallbladder surgery in adults. Excision surgery with Roux-en-Y hepaticojejunostomy is the recommended treatment to prevent complications like cholangitis and cancer.

Chemistry (Weinheim an der Bergstrasse, Germany)·2013

Area of Science:

  • Gastroenterology
  • Surgical Oncology
  • Diagnostic Imaging

Background:

  • Choledochal cysts are congenital bile duct dilatations.
  • Adult presentation often occurs incidentally during cholecystectomy for presumed gallbladder disease.

Observation:

  • Abdominal ultrasonography can misinterpret cysts as gallbladder abnormalities.
  • Preoperative diagnosis relies on ultrasonography and CT to assess porta hepatis structures.
  • Direct cholangiography is crucial for detailed cyst typing and identifying associated anomalies like cholangiocarcinoma.

Findings:

  • Ultrasonography is favored for initial rapid and accurate diagnosis.
  • Cholangiography precisely defines cyst type, strictures, intrahepatic ductal patterns, and filling defects.
  • Adult choledochal cysts carry risks of ascending cholangitis and malignant transformation.

Implications:

  • Early and accurate diagnosis is essential for timely intervention.
  • Surgical excision is indicated to mitigate risks of cholangitis and cancer.
  • Roux-en-Y hepaticojejunostomy represents the definitive surgical management strategy.
