Background:

Purpose of the Study:

  • To evaluate prognostic indicators in patients with cerebral contusions as the only CT lesion.
  • To correlate clinical and radiological findings with patient outcomes.

Main Methods:

  • Retrospective analysis of clinical cases from 1983-1985.
  • Evaluation of patient demographics, Glasgow Coma Scale, epilepsy, focal signs, injury mechanism, fractures, and CT scan findings.
  • Classification of patients into three groups based on consciousness level evolution (no coma, coma with lucid interval, coma without lucid interval).

Main Results:

  • Traffic accidents were the primary mechanism in 75% of severe coma cases.
  • CT scan signs of severity (midline shift, absent basal cisterns/3rd ventricle, multiple lesions) correlated with poor prognosis.
  • Mortality rates were approximately 70%, 80%, and 90% with one, two, or three adverse CT signs, respectively.

Conclusions:

  • Admission CT scan findings are significant predictors of mortality in cerebral contusion cases.
  • The presence of specific adverse radiological signs warrants aggressive management and close monitoring.
