Summary

Flapless dental implant placement is a viable option that reduces postoperative pain in selected patients. More research is needed to determine optimal soft tissue handling techniques for dental implants.

Area of Science:

  • Oral surgery
  • Dental implantology
  • Periodontology

Background:

  • Systematic review of randomized controlled trials (RCTs) on soft tissue management in dental implantology.
  • Searched Cochrane Oral Health Group's Trials Register, CENTRAL, Medline, Embase, and handsearched journals.
  • Contacted authors, an internet discussion group, and 55 manufacturers for unpublished RCTs.

Discussion:

  • Compared flapless vs. flap elevation and crestal vs. vestibular incisions for dental implants.
  • Assessed outcomes including prosthetic/implant failures, aesthetics, complications, pain, patient preference, and mucosal width.
  • Data extraction and quality assessment were performed independently and in duplicate.

Key Insights:

  • Five RCTs involving 140 patients were included.
  • Flapless implant placement and laser exposure significantly reduced postoperative pain compared to flap elevation.
  • No significant differences were found for other analyzed outcomes.

Outlook:

  • Flapless implant placement is feasible and reduces postoperative discomfort in select patients.
  • Insufficient evidence exists to recommend specific incision, suture, or soft tissue augmentation techniques.
  • Well-designed RCTs are required to provide reliable data on optimal soft tissue management for dental implants.
