Summary

A migrated left renal vein stent into the heart was successfully removed via open surgery. A new stent was placed in the left renal vein, with the patient showing excellent long-term recovery and stent patency.

Background:

  • Nutcracker syndrome can necessitate interventions like stenting of the left renal vein (LRV).
  • Stent migration is a rare but serious complication of endovascular procedures.
  • Management of intracardiac foreign bodies requires careful consideration of surgical and endovascular approaches.

Observation:

  • A patient with nutcracker syndrome experienced LRV stent migration into the right ventricle.
  • Initial percutaneous endovascular removal attempts were unsuccessful.

Findings:

  • Successful retrieval of the intracardiac stent was achieved through open cardiac surgery.
  • A new LRV stent was successfully placed via the inferior vena cava (IVC) under direct vision.
  • The patient demonstrated an uneventful recovery and remained asymptomatic at 3-year follow-up.

Implications:

  • This case highlights a rare complication of LRV stenting and successful management strategies.
  • Open cardiac surgery remains a viable option for managing intracardiac stent migration.
  • Successful re-stenting of the LRV with long-term patency is achievable after complex management.
