Summary
Transbronchial needle aspiration (TBNA) in Malaysia is a new but effective diagnostic tool for mediastinal lymph nodes. This study found TBNA to be a safe procedure with a high diagnostic yield for malignancy.
Area of Science:
- Pulmonology
- Thoracic Surgery
- Diagnostic Endoscopy
Background:
- Transbronchial needle aspiration (TBNA) is an emerging diagnostic technique in Malaysia, primarily utilized in specialized respiratory centers.
- Limited data exists on the efficacy and safety of TBNA for mediastinal lymph node assessment in the Malaysian context.
Purpose of the Study:
- To evaluate the diagnostic yield of TBNA for mediastinal lymph nodes.
- To assess the complication rate associated with TBNA at Hospital Tengku Ampuan Afzan (HTAA).
Main Methods:
- A retrospective review of 25 patients who underwent TBNA for mediastinal lymph node evaluation at HTAA.
- Data collection included endoscopy databases, patient records, CT thorax images, and cytological/histological slide review.
Main Results:
- TBNA yielded positive results in 15 patients (60%).
- Overall, 80% of patients undergoing bronchoscopy had confirmed malignancy.
- Bleeding occurred in four patients (16%), with two cases resolving spontaneously and two requiring adrenaline; no mortality was reported.
Conclusions:
- TBNA is a safe and effective procedure for diagnosing mediastinal lymph node conditions in Malaysia.
- The study supports the utility of TBNA in respiratory centers for diagnosing malignancy.

Endoscopic Studies I: Bronchoscopy and Thoracoscopy
Endoscopy is a non-surgical medical technique used to examine a person's internal organs and vessels. This lesson will focus on two types of endoscopic studies: bronchoscopy and thoracoscopy.
Bronchoscopy
Description
Bronchoscopy is a procedure that involves direct visualization of the larynx, trachea, and bronchi for diagnostic and therapeutic purposes. A flexible fiber optic or rigid bronchoscope is used to carry out the procedure. The fiber-optic bronchoscope is more frequently used due to...

Endoscopic Studies II: Thoracocentesis
Thoracentesis(Thoracocentesis), commonly known as pleural tap, is a medical procedure where a 22 gauge needle is inserted into the pleural space, the area between the lung and chest wall. This procedure is commonly performed to diagnose or treat various respiratory disorders.
Description
Excess pleural fluid or air may accumulate in some respiratory disorders in the thoracic cavity. To treat pleural effusion, a physician conducts thoracentesis by carefully piercing the chest wall and entering...
