Summary

Implantable cardioverter-defibrillator (ICD) implantation involves brief ventricular fibrillation (VF) testing. This study found that limited VF induction during ICD procedures does not cause significant neurological or cognitive damage in patients.

  • To assess serum neuron-specific enolase (NSE) levels as a biomarker for cerebral injury post-ICD implantation.

  • 16 patients undergoing initial ICD implantation were assessed.
  • Neurological examinations and cognitive tests were performed pre- and post-procedure.
  • Serum NSE levels were measured at multiple time points: before, during, and up to 48 hours after implantation.

  • No significant differences were found in neurological or cognitive assessments between pre- and post-implantation evaluations.
  • Serum NSE levels remained unchanged from baseline throughout the 48-hour observation period.
  • Patients reported no new neurological symptoms, and examinations revealed no new abnormalities.

Conclusions:

  • Limited induction of VF for defibrillation safety margin testing during ICD implantation appears safe.
  • Brief periods of cerebral circulatory arrest associated with ICD procedures do not lead to detectable permanent neurological injury.
  • Further research is recommended to confirm these findings in larger cohorts.
