Documentation of Nursing Diagnosis01:10

Documentation of Nursing Diagnosis

The nurse documents nursing diagnoses and enters them into the patient record. The identified patient's nursing diagnosis is either written out with a plan of care or entered into the electronic health record.
In some settings, data-driven computerized decision support systems are in place, allowing for more accurate nursing diagnoses. The database within one of these systems includes diagnostic labels defining characteristics, activities, and indicators for nursing. A nurse enters assessment...
Formulating and Validating Nursing Diagnosis I01:26

Formulating and Validating Nursing Diagnosis I

A nursing diagnosis is written when the nurse recognizes a cluster of essential patient data indicating health problems treated with independent nursing interventions. The standardized terminologies of a nursing diagnosis help nurses identify and treat patients' problems. Every electronic health record that uses nursing diagnosis must employ standard diagnostic terminology. Developing an efficient, individualized care plan begins with accurate nursing diagnoses.
There are thirteen domains for...

Formulating and Validating Nursing Diagnosis II

Nursing diagnoses represent a problem validated by major defining characteristics. There are four categories of nursing diagnoses: problem-focused, risk, health promotion or wellness, and syndrome. The anatomy of a nursing diagnosis includes three components: problem statement or diagnostic label, defining characteristics, and related factors.
Risk nursing diagnoses represent clinical judgments of an individual, family, or community more vulnerable to developing the health problem than others...
Diagnostic and Statistical Manual of Mental Disorders (DSM)01:27

Diagnostic and Statistical Manual of Mental Disorders (DSM)

The Diagnostic and Statistical Manual of Mental Disorders (DSM) serves as the primary classification system for mental health disorders, providing standardized diagnostic criteria for clinicians and researchers. First published by the American Psychiatric Association (APA) in 1952, the DSM has undergone several revisions to reflect evolving psychiatric understanding. The fifth edition, DSM-5, released in 2013, introduced key updates that expanded diagnostic categories and modified diagnostic...
Dimensions of Health and Illness01:21

Dimensions of Health and Illness

The factors influencing the health-illness continuum can be internal or external and may or may not be under conscious control. They are related to the following eight human dimensions, and each dimension is interrelated to one other.
Dimensional Analysis01:27

Dimensional Analysis

Dimensional analysis is a valuable technique in fluid mechanics for simplifying complex problems by reducing them into dimensionless groups. These groups capture the essential relationships between the variables involved, allowing researchers and engineers to analyze fluid flow without dealing with each variable individually. This approach reduces the number of independent variables, allowing for easier analysis and better understanding of physical phenomena.
In fluid mechanics, dimensional...

Summary

Current diagnostic systems struggle with inconsistent categories and symptom progression in psychopathology. A shift towards dimensional diagnostics, incorporating patient resources and maintaining factors, is needed for relevant and effective therapy.

Area of Science:

  • Psychiatry and Psychology
  • Mental Health Diagnostics

Background:

  • Categorical diagnostic systems face challenges with inhomogeneous categories and boundary definition.
  • Individual progression of psychopathological phenomena severity complicates traditional diagnostic approaches.

Purpose of the Study:

  • Advocate for a paradigm shift from categorical to dimensional diagnostics in psychopathology.
  • Emphasize the need to integrate pathogenetic and disorder-maintaining factors, including stigmatization and patient significance, into diagnostics.
  • Promote a holistic, person-centered diagnostic approach that incorporates patient resources for improved therapeutic outcomes.

Main Methods:

  • Conceptual analysis of current diagnostic paradigms.
  • Review of factors influencing disorder maintenance and patient experience.
  • Proposal of a dimensional and person-centered diagnostic framework.

Main Results:

  • Dimensional diagnostics offer a more nuanced understanding of psychopathology compared to categorical systems.
  • Integrating patient-reported factors like stigmatization enhances diagnostic relevance.
  • A person-centered approach focusing on patient resources alongside deficits improves therapeutic potential.

Conclusions:

  • A paradigm shift towards dimensional diagnostics is essential for addressing limitations in current categorical systems.
  • Therapy relevancy is increased by considering both etiological and maintaining factors, as well as patient-centered resources.
  • Holistic, person-centered diagnostics foster patient autonomy and well-being by integrating individual strengths.
