Background:

  • Persistent proteinuria exceeding 2 g/day in a 45-year-old male prompted a renal biopsy.
  • The patient presented with no clinical or serological evidence of autoimmune diseases like lupus or Sjögren's syndrome.

Observation:

  • Light microscopy showed focal segmental glomerulosclerosis and diffuse capillary wall thickening.
  • Periodic acid methenamine silver staining revealed a bubbly appearance in capillary walls.
  • Electron microscopy identified numerous 50-70 nm microspheres within a thickened glomerular basement membrane.

Findings:

  • The presence of diffuse microspheres in the glomerular basement membrane is a novel observation.
  • This unique microstructure was found in conjunction with focal segmental glomerulosclerosis.
  • Podocyte foot process effacement and GBM abnormalities were also noted.

Implications:

  • This case expands the understanding of glomerular basement membrane pathology in focal segmental glomerulosclerosis.
  • The identified microspheres represent a previously undocumented ultrastructural finding.
  • Further research is warranted to determine the nature and origin of these microspheres.
