A simplified methacholine challenge test using a single concentration shows similar qualitative results to the standard multi-concentration protocol for diagnosing asthma. This novel approach may offer practical advantages in clinical settings.

Area of Science:

  • Pulmonary Medicine
  • Diagnostic Testing
  • Respiratory Physiology

Background:

  • Methacholine testing is a key diagnostic method for asthma, assessing bronchial hyperresponsiveness.
  • Advancements in dosimeter and software technology offer potential for simplifying methacholine testing procedures.

Purpose of the Study:

  • To evaluate a novel, single-concentration dosimeter protocol for methacholine testing.
  • To compare the diagnostic accuracy and outcomes of the novel protocol against a standard multi-concentration American Thoracic Society (ATS) protocol.

Main Methods:

  • Fifty young subjects with high pretest probability for bronchial hyperresponsiveness participated.
  • Subjects underwent two methacholine challenges in a randomized order: a novel single-concentration protocol and the standard multi-concentration ATS protocol.

  • The novel protocol utilized a fixed methacholine concentration (16mg/mL) with matched nebulization parameters to the standard protocol.
  • Main Results:

    • High rank correlation (0.86) was observed between log slopes of the two protocols.
    • Concordant results for bronchial hyperresponsiveness were found in 46 out of 50 subjects (92%).
    • The novel protocol demonstrated strong agreement with the standard protocol (Cohen's kappa 83%).

    Conclusions:

    • The single-concentration methacholine testing protocol provides qualitative results comparable to the established multi-concentration ATS method.
    • While quantitative differences exist at lower doses, the novel protocol's simplified design and handling present potential benefits for clinical application.
    • This simplified approach warrants consideration for routine asthma diagnosis and management.
