Rare B-cell lymphomas can express the T-cell marker CD3, a finding not previously reported. This highlights the need for comprehensive testing to accurately determine cell lineage in non-Hodgkin lymphomas (NHLs).

  • Non-Hodgkin lymphomas (NHLs) are a diverse group of cancers originating from lymphocytes.
  • Aberrant coexpression of T-cell markers in B-cell lymphomas is known, but typically not CD3.

Observation:

  • This study describes four cases of large B-cell lymphoma that aberrantly coexpressed the T-cell marker CD3.
  • These cases also demonstrated B-lineage markers and clonal immunoglobulin gene rearrangement.

Findings:

  • This is the first reported series of B-cell NHL cases expressing the T-lineage-specific marker CD3.
  • Importantly, these lymphomas did not show clonal rearrangement of the gamma T-cell receptor gene.

Implications:

  • Relying solely on CD3 immunohistochemistry in paraffin sections can lead to misdiagnosis of cell lineage in B-cell NHL.
