Background:

  • Whole craniospinal irradiation in adults is challenging due to the inability to achieve it in a single field at standard treatment distances.
  • Field matching issues and precision are critical concerns in craniospinal radiotherapy.

Purpose of the Study:

  • To introduce and evaluate a novel technique for craniospinal radiotherapy.
  • To minimize field matching problems and maximize precision in adult craniospinal radiotherapy.

Main Methods:

  • A technique involving lateral opposed isocentric fields for the head and a single dorsal field for the spine was developed.
  • An extended source-skin distance of 160 cm and isocentric table rotation were utilized.
  • Field junction edges were shifted to prevent under- or overdosage, with verification using a line laser.

Main Results:

  • The technique demonstrated good homogeneity at the junction between lateral and vertical fields.
  • Reproducibility of table movements was found to be acceptable.
  • The total treatment time per fraction was approximately 15-20 minutes.

Conclusions:

  • The developed craniospinal radiotherapy technique is established and feasible.
  • This method leads to a reduced risk of dose uncertainties.
  • The technique enhances precision in delivering craniospinal radiotherapy.
