Predicting kidney disease progression in Immunoglobulin A nephropathy (IgAN) is challenging. A new risk stratification scheme using six clinical predictors accurately estimates IgAN patient outcomes.

Background:

  • Immunoglobulin A nephropathy (IgAN) is the most common glomerulonephritis.
  • Predicting renal outcome in IgAN patients is difficult, despite progression risk in many.
  • A need exists for a practical risk stratification scheme for IgAN.

Purpose of the Study:

  • To develop a user-friendly risk stratification scheme for IgAN patients.
  • To identify key predictors of renal function deterioration in IgAN.
  • To stratify IgAN patients into distinct risk groups for renal outcome.

Main Methods:

  • Prospective cohort study in 97 Japanese clinical units (from 1995).
  • Analysis of 790 adult IgAN patients with normal baseline renal function.
  • Utilized a decision tree induction algorithm to predict doubling of serum creatinine within 10 years.

Main Results:

  • Severe proteinuria, hypoalbuminemia, and mild hematuria were primary predictors.
  • Serum total protein, diastolic blood pressure, and histological grade were secondary predictors.
  • Six variables stratified patients into seven risk groups (1.0%–51.4% deterioration risk).

Conclusions:

  • Risk of substantial renal deterioration in IgAN is reliably estimated.
  • Six easily obtainable clinical predictors enable valid risk assessment.
  • This scheme aids in clinical routine risk stratification for IgAN patients.
