Beta-cell autoimmunity in pediatric celiac disease: the case for routine screening?

Beta-cell autoimmunity is uncommon in children with celiac disease. Screening for type 1 diabetes is not recommended in this population due to low prevalence.

- 188 young Italian patients with celiac disease were tested for GAD antibodies (GADAs), IA-2As, and IAAs.
- Celiac disease diagnosis was confirmed via jejunal biopsy and antibody testing.
- Patient data included age, sex, pubertal stage, family history, and other autoimmune disorders.
Main Results:
- GADAs were positive in 3.7% of patients; IA-2As in 1.1%. IAAs were negative in all cases.
- No patients developed diabetes during follow-up.
- No significant associations were found between beta-cell autoimmunity and demographic or clinical factors.
Conclusions:
- The prevalence of beta-cell autoimmunity is low in young patients with celiac disease.
- Precocious screening for beta-cell autoimmunity is not supported in this population.
- Further research may explore other autoimmune markers or risk stratification.
