Background:

  • SAPHO syndrome (Synovitis, Acne, Pustulosis, Hyperostosis, Osteitis) is a rare autoinflammatory disorder.
  • It presents with a spectrum of sterile osteitis and neutrophilic dermatosis.
  • Diagnosis can be challenging due to variable clinical manifestations.

Observation:

  • A 62-year-old female presented with a lytic manubrial lesion.
  • Initial differential diagnoses included malignancy and infection, which were ruled out by biopsy.
  • The patient exhibited characteristic features suggestive of SAPHO syndrome.

Findings:

  • Bone scintigraphy showed intense uptake in the affected manubrium.
  • White blood cell imaging demonstrated photopenia at the lesion site.
  • Dual-energy X-ray absorptiometry (DXA) revealed significantly elevated bone density in the lumbar spine.

Implications:

  • This case highlights the importance of considering SAPHO syndrome in patients with unexplained bone lesions and inflammatory signs.
  • Unusual imaging findings, including hyperostosis and altered bone metabolism, are key diagnostic clues.
  • Accurate diagnosis of SAPHO syndrome is crucial for appropriate management and to avoid unnecessary treatments for misdiagnosed conditions.
