Background:

  • Periocular tuberculosis is rare, often presenting after surgical procedures.
  • Eyelid and lacrimal drainage surgeries can potentially trigger localized tuberculous inflammation.

Observation:

  • Four cases of presumed periocular tuberculosis occurred post-blepharoplasty, dacryocystorhinostomy, and lacrimal plug insertion.
  • Diagnosis relied on histopathology, acid-fast bacilli staining, PCR, or therapeutic response.

Findings:

  • All patients received a minimum of 9 months of antituberculosis chemotherapy.
  • Complete recovery was achieved in all four cases.
  • Two patients experienced complications, including ectropion and eyelid retraction.

Implications:

  • Persistent periocular inflammation post-surgery warrants suspicion for localized tuberculosis, especially in endemic regions or with patient history.
  • Clinical diagnosis and therapeutic trials are valuable for presumed eyelid tuberculosis lacking definitive microbiological evidence.
  • Early diagnosis and treatment are crucial to prevent sequelae of periocular tuberculosis.
