Summary

Long-term amantadine use in Parkinson disease can cause reversible corneal edema. Discontinuation of amantadine led to resolution, but permanent cell density reduction may occur.

  • Parkinson disease (PD) management often involves amantadine.
  • Corneal complications are rarely associated with amantadine therapy.

Observation:

  • A 52-year-old female patient with PD presented with bilateral corneal edema.
  • The patient had a 6-year history of amantadine treatment.

Findings:

  • Corneal edema resolved upon amantadine cessation.
  • Endothelial cell density was significantly reduced (
  • Corneal edema recurred upon amantadine re-initiation, resolving again after permanent discontinuation.

Implications:

  • Amantadine can induce reversible corneal edema in PD patients.
  • Prolonged amantadine use may lead to irreversible corneal endothelial cell loss.
  • Ophthalmologists should monitor for corneal changes in PD patients on long-term amantadine.
